* To be eligible for reduced registration fees, registration form and full payment must be received at the CHBA Conference office on or before the specified dates, as shown above.

If paying by Visa or MasterCard, registration form may be faxed.  If paying conference registration fees by cheque, mail registration form with cheque - do not fax.

Delegates are responsible for their own accommodation, transportation and other related expenses.

Registration fee covers all Conference functions as outlined on the CHBA website.

REGISTRATION FEE REFUNDS - CANCELLATION POLICY

  • Cancellations received in writing on or before December 31, 2011 are subject to a $50 handling fee.  
  • Cancellations received from January 1 to February 10, 2012 are subject to a $150 cancellation fee.  
  • Cancellations received after February 10, 2012 or “no show” registrations are not eligible for any refund of registration fees.


NON-MEMBERS
If a non-member registrant becomes a member of the CHBA by April 30, 2012, $100 of the registration fee will be deducted from the national membership fee, upon request.
